A new covered benefit for Medicare patients
Bayview Physicians Group (Bayview) is pleased to share that Medicare has introduced this new program that aims to empower primary care providers in delivering comprehensive and coordinated care, with a focus on enhancing patient outcomes and elevating the overall quality of care. Medicare has added this benefit to enhance the care you receive by focusing on prevention, education communication and coordination for your overall well-being.
In 2012, Bayview adopted the model of Patient Centered Medical Home, a program that began our care delivery transformation. Then, in 2018, we began participating in a Medicare Accountable Care Model to further our care management activities. Now, in 2025, Medicare Part B covers Advanced Primary Care Management services to help your doctor and other health care providers coordinate and tailor care to you needs each month. As a Bayview Physicians Group primary care patient, you may already be benefiting from many of these services. However, it’s important to note that Medicare now covers these services separately from your regular office visits. Bayview primary care providers will be submitting claims on a monthly basis for these services. These services will be administered in the same manner as your regular office visits which includes a 20% cost share. Patients with a secondary plan may have no out of pocket expense, others may have a modest cost share each month, typically between $5 to $10/monthly.

- 24/7 access to your care team for urgent needs
- Communication through Healow, our secure patient portal
- Telehealth visits when deemed appropriate
- Coordination of care with specialists and other healthcare providers
- Access to your individualized care plan created by your care team
- Routine appointment scheduling to monitor your health maintenance and care plan progress
- Keeping you up to date on preventive services such as routine immunizations, medication management, and assessments of your medical and psychosocial needs
- Health reminders for preventive screenings like flu shots, colonoscopies, and more
As a Medicare beneficiary, you do have the option to opt out of the program at the end of any month. If you elect to disenroll, Bayview will assist you in transitioning your care to a practice that is not yet providing these advanced services.
