BAYVIEW PHYSICIANS GROUP ADVANCED PRIMARY CARE JOURNEY

Bayview Physicians Group has been committed to advancing primary care for more than a decade. Our journey began in 2012 with the adoption of the Patient-Centered Medical Home model and expanded in 2018 through participation in an Accountable Care Model to further enhance care coordination and population health management.

Today, our Advanced Primary Care program builds on that experience by providing high-quality, patient-centered care coordination, improved access to healthcare, and proactive support to help you achieve better health outcomes. Our goal is to partner with you to prevent illness, manage chronic conditions, improve communication, and ensure you receive the right care at the right time.

WHAT YOU CAN EXPECT FROM US

  • Monitoring your preventive care needs and recommended screenings
  • Coordinating care among specialists, hospitals, laboratories, pharmacies, and other healthcare providers
  • Communicating with you via our secure Patient Portal
  • Providing 24/7 access to your care team or provider
  • Reviewing medications and helping you manage chronic conditions
  • Following up after hospitalizations, emergency room visits, or significant health events
  • Providing support between office visits through phone calls, secure messaging, and other communication methods
  • Developing and maintaining a personalized care plan based on your health needs and goals
  • Helping connect you with community resources and support services when appropriate

WHAT WE ASK OF YOU AS YOUR HEALTHCARE PARTNER

  • Ensure that your primary care team is your main point of contact for your healthcare coordination.
  • Complete your Annual Wellness Visit and Preventive Screenings
  • Follow your treatment plan and take your medications as prescribed
  • Keep recommended appointments
  • Inform your primary care team of any change in your health status
  • Share your healthcare goals and personal preferences
  • Utilize the Healow patient portal
  • Communicate any changes in insurance coverage or demographic information

FOR MEDICARE PATIENTS

The Medicare APCM program is billed monthly. Services are billed separately and are not billed in association with a visit. The Medicare deductible and cost share/co-insurance applies to these services. Like other Medicare Part B services, APCM is subject to your deductible and a 20% coinsurance. Patients with secondary insurance may have little or no out-of-pocket expense. Please contact your secondary insurance carrier to verify your coverage.

Only one provider may bill Medicare for APCM services during a calendar month. In addition, you have the ability to stop services at any time, effective at the end of the calendar month. If you choose to stop receiving services from Bayview’s APCM longitudinal care program, you may continue to see your Bayview specialists and utilize our episodic primary care services through our Urgent Care, Walk-in clinics, and be seen by our Hospital Medicine team for inpatient services.
